A prospective study of management of inter-trochanteric fractures of femur by modified intramedullary implant

Background: The best surgical strategy for extra-capsular proximal femoral fractures (PFFs) is controversial in the elderly. Poor bone quality and neck screw instability can adversely affect results with currently available fixation devices, which predominantly consist dynamic hip screw-plates reconstruction nails. Hypothesis: lag of modified intramedullary implant achieves better cancellous compaction neck, thereby decreasing risk secondary displacement. Materials Methods: We studied consecutive cases performed between 2019 2021 20 patients (8 females 12 males) a mean age 71.3 ± 9.02 years. All were operated on under hypotensive spinal-epidural anaesthesia supine position traction table. Patients evaluated at pre-op post-op follow-up periods serial radiography clinically by Harris Hip score (HHP) pain disability scoring. Results: Mean was 12.3 7.5 months (4–20 months). Fracture union consistently achieved, after 12.25 1.67 weeks. After last follow-up, 86.8 7.3 (range, 65–100); excellent 11 (55%), good 5 (25%), fair 2 (10%), poor 1 (5%). Conclusion: Modified has advantages simple operation, few complications, clinical efficacy treatment intertrochanteric fractures. However, evaluation its long-term other complications requires large-sample, multicenter observational study.
